Vaccinations

Proof of up to date vaccinations is required. Vaccinations must be administered 7 days prior to your appointment. You must provide a copy, by fax or in person, of up to date vaccination history prior to your appointment. Your vet may be willing to fax us a copy.

For dogs we require proof of up to date:

  • Rabies Certificate
  • DHLPP
  • Bordetella
  • Town License

For cats we require proof of up to date:

  • Rabies
  • FVRCP

Health

All pets must be on an active flea and tick prevention program. All pets must be in good health and have not been ill with a communicable disease within the past 30 days.
